USA - Advocate Aurora
In the United States, a significant proportion of patients with neurocognitive disorders remain underdiagnosed, misdiagnosed or experience delayed diagnosis. Barriers to diagnosing patients include challenges faced by primary care providers (PCP), such as limited time, expertise, and inadequate resources. To address those challenges, Advocate Aurora provided PCPs with education and resources to enhance the efficiency and quality of care for patients with neurocognitive disorders.
Primary care providers received multiple supports for managing patients with abnormal cognitive screens. They had access to brief educational tools on their continuous medical education (CME) website, participated in case-based learning conferences (Project ECHO Memory Care Conference), and were linked to experts from within the health system for consultations (eConsult). Additionally, they had access to simple order sets and templates built into the electronic health record system.

Dr. Gitelman is a behavioral neurologist who serves as the Director of cognitive disorders for the Advocate Medical Group, the Neurosciences Medical Research Director, and the Senior Medical Director of the Advocate Memory Center.

Dr. Michael Malone is an academic geriatrician who serves as Medical Director for Aurora Senior Services and Aurora at Home.

Anna Kallal will be responsible for working with Dr’s Gitelman and Malone to develop educational content, and EHR tools, and participating in educational seminars and training sessions as well as implement expanded use of digital testing and eConsults. Anna will also enhance referral capabilities by seeing some patients.

As of December 12, 2023, 58 providers participated in the project, performing an average of 20 assessments per week. The case-based conferences attracted an average of 25-30 attendees, and there were 26 eConsult requests made by providers.
